I'm fairly sure he was never known as (2). At birth in Genova, he was called Cristoforo Colombo. When he moved to Spain, he adopted Colon as his last name, and may have been known as Cristobal, which looks a bit like (4). I'd guess that Colom (5) is a Portuguese equivalent of Colon. (1) looks suspicious, though.
